Introduction to Tax Iinformation Consent Form

Halt Buzas & Powell, Ltd. (HB&P) has always placed a high value on your privacy and the security of your personal tax information. We have policies in place to safeguard your information while providing the timely and individualized service you deserve.

New federal regulations were recently enacted to ensure that client tax information is disclosed only with the client's explicit authorization.  The regulations require us to obtain your written consent in the attached format before we can disclose your tax information to a third party for any other reason than preparing your tax return.

This means that the consent form is required if you want us to provide a copy of your tax return to your lender, investment advisor or other third party.  We would also need the consent form to contact you with any non-tax information such as HB&P newsletters, event invitations or other HB&P communications.

Below is a link to the consent form for your review.  You may give or deny your consent and you can specify how long your consent lasts.  Please note that you do not have to sign and return the form for HB&P to continue to prepare your tax return.

To give us your consent, please complete the form and return it to our office either by fax or email. If you have any questions about this form, please contact Carol Mount at (703) 836-1350.

Thank you for your assistance as we comply with these new regulations.
